RESUMO

ObjectiveTo determine the situation and challenges of innovation platforms in China, and to explore the construction strategy of Shanghai Nutrition Innovation Platform, which is suitable for Shanghai and may achieve the research and transformation of nutrition innovation and population health, so as to coordinate, unite and gather the superior resources of all parties and promote nutrition innovation. MethodsConstruction scheme and operational mechanism of Shanghai Nutrition Innovation Platform were explored by literature review, expert consultation and questionnaire. ResultsThere were various forms of innovation platforms in China. However, challenges were identified, such as decentralizing force, resource rearrangement and insufficient sharing effect. Shanghai Nutrition Innovation Platform adopted a modular organizational structure, which was divided into central group, node group, and subject group. Shanghai Center for Disease Control and Prevention, as the central organization, is responsible for the platform operation management. The expert database as an academic committee selected key organizations from nutrition-related universities, research institutes, academic associations, centers for disease control and prevention, hospitals and the industry. Based on the opening of its own innovation resources, the platform made effective use of external innovation resources and formed a closely integrated nutrition innovation network of multiple disciplines. ConclusionThis study promotes the construction of innovation platform model of cooperation, co-construction and resource sharing, and provides reference for the construction of innovation platform in China.

Objective To explore the protective effects of Quercetin on hypoxic ischemic brain damage (HIBD). Methods Forty-eight 7-day-old SD rats were randomly divided into sham-operation group, HIBD group and Que treatment group, 16 rats each. HIBD group and Que treatment group were treated by ligation of right common carotid artery to make anoxia and build HIBD model; sham-operation group had the separation of the right common carotid artery only. Que treatment group were injected intraperitoneally with quercetin (40 mg/kg) once a day for 7 days immediately after modeling while sham-operation group and HIBD group received equivalent normal saline at the same time. The rats in each group were scored of neurological function at 1 h after the last administration, and the ability of spatial learning-memory was tested by Morris water maze at the age of 28 days. After performing the test above, all rats were decapitated and the brains were taken. Pathological changes of hippocampus were observed by HE staining; the expression of brain-derived neurotrophic factor (BDNF) and nerve growth associated protein (GAP-43) in hippocampus CA1 area were detected by immunohistochemistry. Results There were significant differences in neurological deficit score and learning-memory ability among the three groups (P<0.01), and neurological deficit score was the highest and the learning-memory ability was the lowest in HIBD group. Pathological examination showed that the structure of hippocampal neurons was intact in sham-operation group. It was loose and disorder, and even loss of neurons in HIBD group. Compared with the HIBD group, the loose in the structure of hippocampal was lighter, and the number of neurons was increased in the Que treatment group. There was statistical difference in the positive expression of BDNF and GAP-43 in the hippocampal CA1 area among the three groups (P<0.01), with those in HIBD group being lower than in Que treatment group and sham-operation group and those in treatment group being lower than in sham-operation group (P<0.01). Conclusions Quercetin can enhance the expression of BDNF and GAP-43 in hippocampal CA1 region, promote nerve regeneration, improve the long-term learning-memory ability of HIBD neonatal rats, and protect the brain.

Objective To explore the effect of Quercetin on the long-term memory and PARP-1/AIF signal path in neonatal rats with hypoxic-ischemic brain damage (HIBD). Methods Fifty-six 7-day-old SD rats were randomly divided into sham-operation group, HIBD group, low dose of Quercetin group (20 mg/kg), and high dose of Quercetin group (40 mg/kg), each of 14 rats. Except for sham-operation group, in the other groups HIBD model were made by right common carotid artery ligation and anoxiate. The Quercetin groups were injected with the corresponding doses of Quercetin immediately once a day continuously for 7 days after the model was made,. Sham-operation group and HIBD group were intraperitoneally injected with normal saline at the same time. Neural function was evaluated by Hanging wire test and Vertical pole test at 21 days old. The capacity of learning and memory was detected by Morris water maze at 28 days old, and then rats were killed and brains were taken. HE was used to observe the pathological changes of hippocampus. Western blot were used to detect the expression of PARP-1 and AIF in hippocampus. Results Compared with sham-operation group, the neural function and learning and memory ability decreased significantly in HIBD group. Those ability in both low dose and high dose of Quercetin groups were remarkably increased in comparison with HIBD group, and there were statistic differences (P??0.05). Conclusion Quercetin could improve long-term learning memory in newborn rats with HIBD, and the mechanism may be down-regulation of PARP-1/AIF cell apoptosis signaling pathway, inhibition of neuronal apoptosis, and thus play a role in protection of brain.

Objective To analyze the treatment effect and factors affecting the prognosis in young people of papillary thyroid carcinoma (PTC).Methods A total of 69 PTC patients were reviewed retrospectively in Sun Yatsen University Cancer Center from Dec 1996 to Oct 2005.The prognostic status were compared using the Kaplan-Meier methods,the factors were analyzed by Log-rank test and the multiple factors were estimated by the Cox regression models.Results There were 26 males and 43 females.The median age was 24 years (mean:23.1 years).The average follow-up time was 113 months ranging from 73 to 174 months.20.3% patients (14/69) experienced recurrence during follow-up time and 1.4% patient (1/69) died.All the patients received levothyroxine after operation.The 10 year disease-free survival (DFS) rate of patients with or without extra thyroidal extension (ETE) of PTC was 29.6% and 80.1%,respectively (P < 0.01).Multivariate analysis showed that young PTC people with ETE had a worse prognosis (P =0.004).Conclusions Young papillary thyroid carcinoma patients had a favorable outcome.Surgical procedure was the first choice for this kind of patients.ETE was an independent factor for the recurrence of young PTC patients.

Objective To discuss differentiated thyroid carcinoma (DTC)postoperative lymph node metas-tasis and clinical use and the effect of 131 I treatment.Methods to185 patients with lymph node metastasis of DTC were selected by random sampling method.The standard 131 I treatment was administrated,ultrasound,CT and Tg chan-ges and imaging characteristics of 131 I treatment were evaluated the clinical curative effect was determined.Results Imaging changes:the whole group of patients,153 patients (82.7%)were effective.153 patients with effective treat-ment,105 cases examined LMDTC completely eliminate (56.8%),48 cases examination revealed narrowing (25.9%).Mere lymph node metastasis after treatment was better than that of patients with viscera metastasis,imaging examination results were statistically significant differences (χ2 =2.57,P =0.02).After treatment,imaging examina-tion result in patients with smaller lymph node was superior to that in the patients with relatively large diameter lymph nodes (P >0.05).Tg level changes:there was a significant reduction in the Tg 97 cases of 185 patients (51.1%), and general decline in 54 patients (29.2%).Tg improvement effect of patients with mere lymphatic metastasis after treatment was better than other viscera metastasis in patients with statistically significant differences (χ2 =3.41,P =0.01).Lymph node Tg improved after treatment in patients with small diameter,which was better than that with other viscera metastasis patients with statistically significant differences(χ2 =6.34,P =0.02).Conclusion Comprehensive curative effect of 131 I treatment of lymph node metastasis postoperatively in patients with DTC is distinct,and is worth popularizing in clinical use of DTC.

<p><b>OBJECTIVE</b>To investigate the number of dissected lymph nodes and the incidence of injury to parathyroid glands during surgery for papillary thyroid carcinoma (DTC) in young patients.</p><p><b>METHODS</b>This study collected clinicopathological data of 51 young patients with PTC. Of the 51 patients, 18 patients were classified into carbon nanoparticles group (CNP group) and 33 patients into traditional surgical group (TS group). The number of dissected lymph nodes and the incidence of injury to parathyroid glands were analyzed using Wilcoxon rank sum test and chi-square test.</p><p><b>RESULTS</b>There were 16 males and 35 females, with a male/female ratio of 1: 2.19. The age ranged from 14 to 29 (25 ± 3.9) years. There was no statistically significant difference in age, gender, T-classification, TNM stage and surgical procedures between two groups (P > 0.05). The total number of dissected lymph nodes in CNP group was higher than that in TS group (Z = -2.258, P < 0.05) . However, significant difference in the total number of metastatic lymph nodes between the two groups was not found (Z = -0.396, P > 0.05). In level VI, the detected lymph node number of group CNP was higher than that of TS group (Z = -2.461, P < 0.05) but there was no significant difference in the detected number of metastatic lymph nodes (Z = -1.396, P > 0.05) . The rates of injury to parathyroid gland were 5.5% in CNP group and 18.2% in TS group, respectively (χ(2) = 1.568, P > 0.05).</p><p><b>CONCLUSION</b>Carbon nanoparticles could be an effective lymph nodes tracer applying to PTC operation in young patients.</p>

ObjectiveTo evaluate the clinical value of infrahyoid myocutaneous flap in the reconstruction of mouth floor tissue defects. MethodsInfrahyoid myocutaneous flap was made including stenohyoid,thyrohyoid,omohyoid and the affiliated skin based on the axis of superior thyroid artery,vein and ansa cervicalis. The clinical data of 11 patients with reconstruction of mouth floor tissue defects using infrahyoid myocutaneous flap (maximum area was 8 cm × 3 cm) from Match 1993 to June 2009 were retrospectively reviewed.ResultsSeven infrahyoid myocutaneous flaps which reserved platysma branch of superior thyroid artery were all alive, while 2 patients presented epidermal necrosis of 4 un-protected patients. The follow-up period was range from 24 months to 195 months, one patient received salvage surgery for local regional lymphatic recurrence in level Ⅰ,which survived without diseases until now.The rest cases had turned out a success.No dysphagia,dysphonia,and tongue extension difficulty was reported in any patient.ConclusionInfrahyoid myocutaneous flap is one of the valuable donor sites for the reconstruction of mouth floor tissue defects after radical oncologic operations,especially for those with early stage carcinoma of the mouth floor.

Objective To investigate the treatment and prognosis of patients with papillary thyroid microcarcinoma. Methods The clinical and following-up data of 124 patients with papillary thyroid microcarcinoma treated at the Department of Head and Neck Surgery of Cancer Centre, SUN Yat-sen University from Jan 1990 to Dec 1999 were analyzed retrospectively for mortality and survival rate ( KaplanMeier). A multivariate analysis was performed in these patients by Cox proportional hazard model. Results The overall 10-year and 15-year survival rate of all 124 patients with papillary thyroid microcarcinoma were 94.9% and 92.5% respectively. The univariate analysis showed the prognostic factors significantly influencing the survival of patients included age (being worse for those of 35 years and older) at presentation, and the status of distant metastasis ( all P < 0. 05 ) , while gender, incidentally found at surgery, the size of primary tumor, the status of neck lymph node metastasis, recurrence after a primary resection, and the extent of surgical resection did not significantly effect the prognosis. Multivariate analysis showed the age more than 35 years at presentation was an independent risk factor indicating worse prognosis (P = 0. 045 ). Conclusioas The prognosis of patients with papillary thyroid microcarcinoma is satisfactory, but that is poor for those patients 35 years old and up at presentation and with distant metastasis.

OBJECTIVE@#The current study was designed to examine the expression of Skp2 gene in laryngeal squamous cell carcinoma (LSCC) and to investigate the role of Skp2 gene in tumorigenesis and progression of LSCC.@*METHOD@#FQ-PCR method was used to examined the expression of Skp2 gene in 40 LSCC and 10 normal laryngeal mucosa tissues, and relationship between its expression and clinical biological factors of patients with LSCC was analyzed.@*RESULT@#The median copy number of Skp2 mRNA expression in LSCC was 6622.54 copy/microg RNA, the median copy number of Skp2 mRNA expression in normal laryngeal mucosa tissues was 0 copy/microg RNA, there was a very significant difference between them (P < 0.01); The positive rate of Skp2 mRNA expression in LSCC and adjacent normal laryngeal tissue were 50%, 0, respectively (P < 0.01). The median copy number of Skp2 RNA expression in LSCC with cervical lymph node metastasis was 617138.4 copy/microg RNA, the median copy number of Skp2 mRNA expression in LSCC without cervical lymph node metastasis was 0 copy/microg RNA, there was a very significant difference between them (P < 0.05); The positive rate of Skp2 mRNA expression in LSCC with and without cervical lymph node metastasis were 100.00%, 35.48%, respectively (P < 0.01).@*CONCLUSION@#Skp2 gene might have relation with the cervical lymph node metastasis of LSCC. FQ-PCR is an accurate assay to detecting expression of Skp2 mRNA in patient with LSCC. The level of Skp2 mRNA expression might be a new and more accurate marker, and it can be used to predict cervical lymph node metastasis of LSCC.

OBJECTIVE The purpose of this studywas to investigate the correlation of E-cadherin(E-cad) and ?-catenin(?-cat) expression with clinical factors and prognosis in laryngeal squamous cell carcinoma (LSCC) patients. METHODS The expression of E-cad and ?-cat in 79 cases of LSCC and 10 cases of adjacent normal laryngeal mucosal tissues were evaluated by SP immunohistochemical methods. RESULTS All 10 normal samples were positive for expression of E-cad and ?-cat , The positive expression of E-cad and ?-cat in LSCC were 34.18 % and 40.51 % respectively. RESUMO

Objective To study the differences between diet induced obesity (DIO) and diet induced obesity resistance (DIO-R) during insulin resistance (IR) in rats. Method Male SD rats were fed by high-fat diet 1 (45 % fat kJ) after adaptation. At 3rd week, rats were divided into 3 groups according to their body weight: rats belonging to the middle 1/3 of their body weights were the control group, and was fed by control diet (10 % fat kJ). The heaviest 1/3 and the lightest 1/3 belong to DIO and DIO-R group, respectively, and were fed by high-fat diet 2 (60 % fat kJ). Five rats were selected randomly from each group and sacarified at 3rd, 6th, 8th, 10th and 11th week. The energy intake, body fat contents, fasting blood glucose (FBG), serum lipid, serum insulin, insulin sensitive index(ISI)and leptin of rats were monitored dynamically. Results DIO rats were significantly different from DIO-R and control rats in energy intake, body fat content, FBG, lipid, serum insulin and leptin. Conclusion Fed by the same high-fat diet, DIO rats are more sensitive than DIO-R rats to become IR.
